Case Description:

    Fig 1 shows the chest radiograph of a 70-year-old woman on postoperative day 4 following right upper lobectomy for pathologic stage T2, N0, M0 bronchioloalveolar carcinoma of the lung. Clear surgical margins were obtained at operation, and the patient was experiencing an uneventful postoperative course.

 The most likely chest radiograph diagnosis is:

    1. right apical pneumothorax

    2. postoperative changes related to a pleural tent procedure

    3. residual lung tumor

    4. hydropneumotherax
